    How does a DB system decide when create a certain index ? Does it happen the first time some search is executed ?

    The DBMS doesn't decide that, the user does when creating the table (or later).

    Yes, the indexing make the alteration is too costly, sometimes i would prefer add a new rows first and deleted the old one, it makes the altering quicker.
